1. Our Services
Pemberton Physiotherapy & Wellness provides comprehensive healthcare services including:
- Physiotherapy
- Registered Massage Therapy
- Chiropractic care
- Acupuncture (Traditional Chinese Medicine)
- Osteopathic treatment
- Foundation Training
- Craniosacral therapy
- Pilates
All services are provided by licensed healthcare professionals working within their regulated scope of practice.
2. Appointment Booking and Cancellation
Online Booking:
- Appointments can be booked online through our Jane App booking system
- You must provide accurate information and create a patient account
- Confirmation will be sent via email or SMS
Cancellation Policy:
- Cancellations must be made at least 24 hours in advance
- Late cancellations or no-shows may result in cancellation fees
- Emergency cancellations will be considered on a case-by-case basis
Rescheduling:
- Appointments may be rescheduled subject to availability
- We reserve the right to reschedule due to practitioner availability or unforeseen circumstances
3. Payment Terms
Payment Methods:
- We accept cash, debit cards, credit cards, and electronic transfers
- Online payments can be made securely through Jane App
- Payment is due at the time of service unless prior arrangements have been made
Insurance:
- We provide direct billing to many insurance providers
- Patients are responsible for understanding their coverage
- Any unpaid balances after insurance processing are the patient’s responsibility
Outstanding Balances:
- Outstanding balances must be paid within 30 days
- Interest and collection fees may apply to overdue accounts
4. Patient Responsibilities
Medical Information:
- Provide accurate and complete medical history
- Inform practitioners of any changes to medications or health conditions
- Follow prescribed treatment plans and home exercise programs
Conduct:
- Treat all staff and other patients with respect
- Arrive on time for appointments
- Follow clinic policies and procedures
Personal Property:
- We are not responsible for lost or stolen personal items
- Please secure valuable items before treatment
5. Professional Boundaries and Consent
Professional Relationships:
- Our practitioners maintain professional relationships with all patients
- Inappropriate behavior will not be tolerated
Treatment Consent:
- Informed consent is required for all treatments
- Patients have the right to refuse treatment
- Minors require parental/guardian consent
6. Privacy and Confidentiality
- We protect your personal health information in accordance with applicable privacy laws
- Information is only shared with your consent or as required by law

- Consent will be obtained before using patient images or testimonials
7. Liability and Treatment Risks
Treatment Risks:
- All medical treatments carry inherent risks
- Practitioners will discuss potential risks and benefits
- Patients assume responsibility for following post-treatment instructions
Limitation of Liability:
- Our liability is limited to the extent permitted by law
- Professional liability insurance is maintained by all practitioners
Emergency Situations:
- We are not equipped to handle medical emergencies
- In emergency situations, call 911 or go to the nearest emergency room
8. Website and Jane App Use
Acceptable Use:
- Use the website and Jane App booking system for lawful purposes only
- Do not attempt to gain unauthorized access to our systems
- Maintain the confidentiality of your login credentials
- Do not share your patient account access with others
Jane App Integration:
- Our online booking system is powered by Jane App
- Jane App is PIPA and PIPEDA compliant for healthcare data protection
- By using our online booking system, you agree to Jane App’s terms of service
Content:
- Website content is for informational purposes only
- Content should not replace professional medical advice
- We strive to keep information current but cannot guarantee accuracy
9. Communication
Electronic Communication:
- Email and text communications are not secure
- Confidential information should be discussed in person or over the phone
- We may send appointment reminders and general information electronically

- Notify us immediately of any changes
10. Termination of Services
Patient Termination:
- You may terminate our services at any time
- Outstanding balances must be paid upon termination
Clinic Termination:
